Expanding on the Nirvana Nevermind info, there are actually three versions of the CD. Two don't have the bonus track, and only one gives the Swimmer.
The first printing of the CD mistakenly didn't include the track Endless, Nameless. There were 50,000 of these. They will not give you a swimmer.
The second pressing (and somewhat beyond, I guess) added the song as a bonus track, at the end of Something In the Way. This also does not give the Swimmer.
At some point they dropped the bonus track, and this is the version that will give you the Swimmer.

(I think the Catalog and Label info is the same on all versions, not sure about the UPC.)
If you look on the silver side of the cd and find the matrix code (letters and numbers etched into the cd around the center hole) the first pressing reads "38 DGCD 24425-2 SRC+01".
I have no idea what code the copy that makes a Swimmer has, but if yours has the above matrix code or Endless, Nameless, it's useless (for MR2, anyway.)(The first pressing gives you a Tiger/Golem)
